We are seeking someone to join our dynamic team as a Case Manager at Ascension Place. The purpose of this role is to provide case management services to women dealing with chemical dependency and/or mental health diagnoses and to provide leadership regarding mental health services for residents and participate in the day-to-day operations of the transitional housing program. This role will require hours Sunday through Thursday with Fridays/Saturdays off (four day shifts and one evening shift).

Provide leadership regarding mental health services for all residents:

  • Continually develop and maintain current knowledge about research, resources and information related to mental health and communicate this information to the team and to residents
  • Collaborate with the Program Director to ensure mental health services meet resident needs and are compatible with agency policy and mission
  • Provide leadership and support to team members especially related to residents with mental health needs
  • Coordinate the mental health services for residents on own caseload as well as collaborating effectively with other case managers service residents with mental health needs
  • Develop and maintain relationships with mental health service providers in the community fostering continuity of care and promoting Ascension Place
  • Facilitate mental health groups which promote educated mental health consumerism and resident self-awareness and other meetings that arise
  • Provide leadership and staff trainings regarding crisis intervention, and collaborate with the Program Director to manage crisis intervention policies and practices

Provide case management services to assigned residents (up to 16) living in transitional housing:

  • Conduct individual weekly meetings with each resident on caseload
  • Maintain communication and coordinate services with interdisciplinary team members such as county social worker, day treatment therapist, psychiatrist and others
  • Assist residents with paperwork, referrals (to appropriate community services) and attend appointments/meetings as needed
  • Complete a trauma assessment, develop coping, goal, safety and abuse prevention plans with residents on caseload
  • Maintain resident files so that they are organized and current at all times with weekly progress notes and updated case plans
  • Ensure residents meet financial requirements by assisting them with financial paperwork and communicating with Program Director

Participate as an Ascension Place team member in the day-to-day operations of the program:

  • Provide front office coverage, which includers answering phones and doors, responding to immediate resident concerns, documenting medication and keeping work area neat/organized
  • Assist all residents with day-to-day needs including support, problem solving, conflict resolution, crisis intervention and other concerns
  • Maintain effective team communication by logging about daily contact with residents
  • Maintain the security of the building and ensure safety of residents and complete incident reports as necessary

Qualifications

  • Bachelor’s or Master’s Degree in social service/administration or equivalent
  • Experience working with women of diverse racial and socioeconomic backgrounds, preferably in a housing and/or nonprofit organization
  • Experience with Gmail, Google apps and Microsoft Office
  • Excellent communication skills (both written and verbal)
  • Criminal/driving background check will be required (must have a good driving record and valid auto insurance)
